College for Seniors Forms

• Request for CFS Guest Pass
• Spring 2026 Drop Request: Submit a drop request for each course; or, drop all courses and request a refund by Friday, March 20, 3 p.m.
• Spring 2026 Request to Apply Tuition Credits
• Spring 2026 Scholarship Request: Requests for partial scholarships are due by Friday, February 13, noon during lottery registration; and by Monday, March 16 during open registration.
• Fall 2026 Course Proposal (September 21-November 13, 2026): Course proposals will be accepted March 1-May 1, 2026. Preview a sample form.
• Summer 2026 Course Proposal (June 15-July 24, 2026): Course proposals were accepted December 1, 2025-February 1, 2026.
